Sales Use Registration Certificate

Unemployment Insurance Tax
Almost all non-agricultural companies in Arizona are required to pay unemployment insurance tax to protect their workers during unemployment. Contact the Department of Economic Security at 628-6210.

Workmen’s Compensation
Nearly all employers doing business in Arizona must obtain worker’s compensation insurance. Contact your insurance agent or the Industrial Commission at 628-5188.

Registrar of Contractors
All contractors doing business in Arizona must be licensed and meet certain bonding requirements. Call 628-6345.

Liquor License
Applications from establishments to be located within the city limits will be reviewed by the City Council, while county applications will be reviewed by the Board of Supervisors. Call 628-6595. Both city and state liquor licenses are required of establishments located within incorporated areas. Cost will vary according to type of license.

Formation of a Corporation
Contact the Arizona Corporation Commission at 628-6560 if you are considering forming an Arizona corporation. It is necessary to check with the Commisssion regarding your business name, filing of “Articles of Incorporation” and annual reports. The Arizona Corporation Commission-Utilities Division, can also assist consumers in finding the utility that serves your area. The Utilities Division may be contaced at 628-6550.
